Every year over 100,000 Australians are hospitalized due to problems with their medication, of which over half of these could have been avoided. A home medication review (HMR) is a free service funded by the Australian Government with the objective of helping to prevent these medication mishaps.

A pharmacist will visit you in the comfort of your own home or a location of your choice, to talk to you about your medications. As part of a focused team of professionals, they will liaise with your doctor and cover various topics such as:

• How to your medications to ensure you get the best benefit.
• What to expect when taking them including what side effects that need to be reported to your doctor.
• Explain when to take your medicines and why they are needed.
• Check that the prescription medications, vitamins and over the counter medicines are safe and appropriate to take together.
• Explain generic medicines to minimize confusion
• Explain possibility of simplifying medication regimes.
• Explain how to store them properly.
